Following reports, Ladysmith residents will soon benefit from vital medical services, as the Transnet Phelophepa Healthcare Train prepares for an 11-day stop in the town.
According to information provided by Transnet and its partners, the Phelophepa Healthcare Train—meaning “good, clean health” in Sesotho and Setswana—was launched in 1994 as a modest three-coach mobile eye clinic.
Over the past three decades, the initiative has expanded into two fully equipped 19-coach trains, Phelophepa I and II, delivering primary healthcare services to underserved and rural communities across South Africa.

Transnet reports that the programme has assisted an estimated 15.8 million people through on-board clinics and outreach initiatives. Operating for approximately 35 to 36 weeks each year, the trains visit around 70 communities annually, serving roughly 400,000 patients while supporting the Department of Health in areas where access to medical care remains limited—often with ratios as high as one doctor per 5,000 residents.
The Alfred Duma Municipality Communications Unit has confirmed that the train will be stationed at Ladysmith Train Station from 19 January to 30 January 2026, with services available daily between 7:30am and 4pm.
“Residents are invited to free healthcare screenings, including dental, cancer and diabetes checks, eye care and counselling services,” the Communications Unit said. Members of the public are encouraged to bring their clinic cards, as well as children’s immunisation cards, when attending.
Transnet further confirmed via its official communication platforms that the Phelophepa trains will resume operations nationwide on 18 January 2026 following routine maintenance, aligning with the Ladysmith schedule.
Services offered on board the train:
Unlike conventional outreach programmes that rely on referrals, the Phelophepa Healthcare Train delivers a wide range of services on-site, either free of charge or at minimal cost, enabling patients to receive immediate care.
Services available during the Ladysmith visit include:
- Dental care, including extractions, fillings and cleaning, at R10 per procedure
- Vision screening at no cost, with prescription glasses available for R30 per pair
- General health assessments, including diabetes and cancer screening
- Pap smears and counselling services
- Pharmacy services, providing essential medication at R5 per patient
- Health education sessions focusing on hygiene, disease prevention and wellness
- Psychological support services for mental health concerns
Qualified healthcare professionals staff the train, with several services—such as eye testing and glasses production—completed on-site, reducing the need for follow-up visits.
Services will be offered on a first-come, first-served basis, and no advance bookings are required. Residents are advised to arrive early due to expected demand. While not compulsory, bringing identification, medical records and existing prescriptions is recommended to assist medical staff.

The Phelophepa initiative holds particular importance within KwaZulu-Natal’s uThukela District, which includes Ladysmith under the Alfred Duma Local Municipality.

The programme operates in partnership with organisations such as Colgate, which supports oral health education, and Roche, which provides diagnostic support. These collaborations enhance service delivery while keeping costs low for patients.
