A head-on collision along the R34, just outside Vryheid near Hennie’s Garage, claimed the lives of two people, including a 10-year-old child, on Monday afternoon, 12 January 2026. Several others were left with life-threatening and serious injuries following the high-impact crash.

KwaZulu Private Ambulance (KZPA) spokesperson Craig Botha confirmed that emergency services were dispatched to the scene at approximately 13:30 after reports of a severe collision involving multiple vehicles.

After arriving, paramedics initiated a triage process to identify and prioritise patients in critical condition. Emergency teams then worked to stabilise all injured occupants before arranging urgent transport to medical facilities for specialist care.

“Upon arrival, first responders were met with a scene of absolute devastation. Two passenger vehicles had collided head-on with such force that the wreckage left the roadway partially obstructed. Paramedics immediately began a rapid triage process to assess the occupants of both vehicles,” Botha said.

Tragically, one adult male sustained catastrophic injuries and was declared deceased at the scene. “Despite the swift arrival of emergency teams, he had already succumbed to his injuries,” Botha confirmed.

The collision also resulted in multiple critical and serious injuries among survivors, including four young children, underscoring the severity of the crash.

Among the injured were two girls aged between five and ten years who were found to be in a critical condition, requiring immediate Advanced Life Support intervention. A three-year-old girl and three adults sustained serious injuries. All patients received on-site medical care before being evacuated by ambulance.

Botha further explained that, once all patients had been stabilised, they were transported to the nearest medical facilities. However, the 10-year-old girl later succumbed to her injuries shortly after arrival at hospital.

In managing the scene, the R34 was initially closed to all traffic to allow emergency personnel to operate safely. While the road has since been reopened to a single lane, significant delays persisted as authorities worked to clear debris and damaged vehicles.

The South African Police Service (SAPS) remains on site and has launched a formal investigation into the cause of the collision, with officers working to establish the exact circumstances that led to the crash.

Notably, this incident marks the second serious collision on the R34 within a 24-hour period. Botha confirmed that on Sunday night, 11 January 2026, KwaZulu Private Ambulance was dispatched to another high-priority incident near Gluckstadt following reports of a severe vehicle rollover.

“Upon arrival, emergency first responders were met with a chaotic and harrowing scene. A single bakkie transporting passengers home from a soccer match had reportedly lost control, veering off the roadway and overturning multiple times, leaving occupants scattered across the road,” Botha said. Paramedics immediately implemented a mass-casualty triage protocol due to the high number of injured individuals.

Following assessments, six occupants were found to have sustained serious injuries requiring Advanced Life Support intervention, while a further 17 occupants suffered minor injuries.

“The complexity of the scene required seamless coordination among multiple responding units to stabilise all 23 patients before transporting them to various hospitals for definitive care,” Botha added.

SAPS was present at the scene and will conduct a full investigation into the cause of that incident as well.
